Mesalzer Tablets Mesalzer contains Mesalamine (5-aminosalicylic acid / 5-ASA) in a prolonged-release formulation. Mesalamine acts locally on the colonic mucosa to diminish inflammation by inhibiting cyclooxygenase (COX) and lipoxygenase (LOX) pathways, thereby reducing the biosynthesis of pro-inflammatory prostaglandins and leukotrienes (specifically $LTB_4$). Additionally, it acts as a scavenger of reactive oxygen species (ROS) and modulates nuclear factor kappa B ($NF-\kappa B$) activation within the intestinal epithelium. The prolonged-release formulation ensures targeted delivery of the active drug throughout the distal small intestine and colon while minimizing systemic absorption. Mesalzer is indicated for the induction and maintenance of remission in patients with mild-to-moderate active ulcerative colitis and for the management of Crohn's disease involving the terminal ileum and colon.
