In autoimmune conditions, it accumulates within intracellular lysosomes and raises the endosomal/lysosomal pH, thereby interfering with antigen processing, inhibiting major histocompatibility complex (MHC) class II presentation, and suppressing Toll-like receptor (TLR) signaling (specifically TLR7 and TLR9). This leads to decreased activation of dendritic cells and reduced production of pro-inflammatory cytokines such as interleukin-1 (IL-1), interleukin-6 (IL-6), and tumor necrosis factor-alpha (TNF-alpha). As an antimalarial, it concentrates in erythrocyte digestive vacuoles of Plasmodium parasites, preventing the polymerization of toxic heme into inert hemozoin, resulting in parasite cell destruction. HQKAST-200 is indicated for the treatment of rheumatoid arthritis, systemic lupus erythematosus (SLE), discoid lupus erythematosus, and uncomplicated malaria caused by susceptible strains.
